چکیده
Results 156 patients were included in an intention-to-treat analysis. Th ree patients withdrew from the trial before fi nishing follow-up. Recurrent ulcer bleeding within 30 days was 10.3% in the aspirin group and 5.4% in the placebo group (diff erence, 4.9 percentage points [95% CI, −3.6 to 13.4 percentage points]). Patients who received aspirin had lower all-cause mortality rates than patients who received placebo (1.3% vs. 12.9%; diff erence, 11.6 percentage points [CI, 3.7 to 19.5 percentage points]). Patients in the aspirin group had lower mortality rates attributable to cardiovascular, cerebrovascular, or gastrointestinal complications than patients in the placebo group (1.3% vs. 10.3%; diff erence, 9 percentage points [CI, 1.7 to 16.3 percentage points]).

SECONDARY PREVENTION OF UPPER GASTROINTESTINAL BLEEDING IN LOW DOSE ASPIRIN USERS INFECTED WITHHELICOBACTER PYLORI
ABSTRACT Background: Aspirin even at low dose ( 100 mg) can increase the risk of upper gastrointestinal bleeding. It is usual to use low dose aspirin for cardiovascular prophylaxis. We hypothesized that Helicobacter pylori eradication is as effective as omeprazole maintenance therapy for secondary prevention of bleeding in those who take low dose aspirin and are positive for Helicobacter pylor...
متن کاملMortality with upper gastrointestinal bleeding and perforation: effects of time and NSAID use
BACKGROUND Some people who suffer an upper gastrointestinal bleed or perforation die. The mortality rate was estimated at 12% in studies published before 1997, but a systematic survey of more recent data is needed. Better treatment is likely to have reduced mortality. An estimate of mortality is helpful in explaining to patients the risks of therapy, especially with NSAIDs. METHODS A systemat...
متن کاملUpper Gastrointestinal Bleed Induced By Non-Steroidal Anti-Inflammatory Drugs/Aspirin In Jamaica
Background: Gastrointestinal disturbances are the most common adverse effects of NSAIDs and ASA. Upper gastrointestinal bleeding (UGIB) occurs in over 2-4% of NSAIDs/ASA users. Objectives: To review patients admitted with upper gastrointestinal bleed induced by NSAIDs/ASA at the University hospital of the West Indies between January 2001 to December 2007. Methods: Patients admitted between Janu...
متن کاملOptimal timing of vitamin K antagonist resumption after upper gastrointestinal bleeding. A risk modelling analysis.
The optimal timing of vitamin K antagonists (VKAs) resumption after an upper gastrointestinal (GI) bleeding, in patients with continued indication for oral anticoagulation, is uncertain. We included consecutive cases of VKA-associated upper GI bleeding from three hospitals retrospectively. Data on the bleeding location, timing of VKA resumption, recurrent GI bleeding and thromboembolic events w...
متن کاملRecommencing aspirin following a peptic ulcer bleed: when is the time right?
As the population ages it is increasingly common for patients who present with a peptic ulcer bleed to have concomitant cerebrovascular or cardiovascular disease. Traditionally antiplatelet therapy, including aspirin, is stopped for a period of 4-8 weeks following an acute bleed although this approach is not evidence based. A study by Sung et al [1] addressed the issue of what to do with aspiri...
